How to refer to the eclinic
- All chronic wounds should be assessed using the 9SWAT – 9 Step Wound Assessment Tool, prior to referring to Tissue Viability
- All priority cases will be replied to within 2 working days and non priority cases replied to within 10 working days
- Please complete the e-clinic referral form. Attach, with clear images and send together to the email address above.
- Incomplete referrals will be returned with request for completed referral form and/or images
